We use the Ottawa Ankle Rules — specific clinical criteria — to determine if an X-ray is needed to rule out fracture. We assess this at your first session and refer you for imaging if indicated.

For moderate (Grade 2) and severe (Grade 3) sprains, a walking boot for 1–2 weeks reduces pain and protects the ligament. We advise the appropriate brace and when to progress to shoes.

Recurrent ankle sprains are almost always due to inadequate rehabilitation of the first sprain — specifically, incomplete restoration of proprioception (joint position sense). Our programme specifically targets this.

Grade 1 sprains: 1–2 weeks. Grade 2: 3–6 weeks. Grade 3: 6–12 weeks. Return to sport is based on passing objective tests (figure-of-8 run, hop tests), not just pain resolution.

Ice in the first 48–72 hours to reduce swelling. After 72 hours, contrast bathing (alternating warm and cold) can be used. We provide specific guidance on ice application to maximise benefit.

Gentle walking within pain tolerance is encouraged from day 1 for mild-moderate sprains. Complete non-weight-bearing is rarely necessary and delays recovery.

Only if rehabilitation is incomplete. Full proprioception and strength restoration through physiotherapy gives you an ankle as strong as — or stronger than — before the injury.

Surgery for ankle sprains is rarely needed even for Grade 3 (complete) ligament tears. Non-surgical rehabilitation has equivalent long-term outcomes to surgery for most patients.
